Primary Care Clinician Learning Collaborative Curriculum

Physician Champion quarterly meetings are used to review data dashboards and provide opportunity for quality improvement planning. Participating clinicians didactics are offered eight months of the year, with two repeating options in each offering month. Clinical teams receive periodic email fun facts and have access to an electronic repository of resources to facilitate clinical care and navigation of services. CME credits are provided. MOC projects available for those with interest.
