Step 1: Complete the Program Assessment Form (PAF).
Applicants should complete and send the Program Assessment Form with a copy of all previous college or university transcripts by email to IUHTinfo@iu.edu (unofficial versions are accepted at this stage of the process). The program will evaluate the applicant’s academic background and determine if they qualifies for admission to the program. After the candidate receives an approval to apply letter, they can progress to Step 2.
Step 2: Obtain the Approval to Apply Email.
Before submitting the university application and the program’s supplemental forms, you should have already completed a Program Assessment Form and received an Approval to Apply Email. Contact IUHTinfo@iu.edu if you have not received your approval email.
Step 3: Complete the Free Online University Application.
Following the instructions in the approval to apply email, complete the online application. Refer to your approval to apply letter for your specific term and program code information.
Step 4: Submit Official Transcripts.
As part of the IUPUI application process, applicants must submit official copies of their prior college or university transcripts. Transcripts previously submitted to IUPUI Office of Admissions cannot be used.
Step 5: Check your IU Email.
When you submit the university application you will be instructed to create an IU email account. Check this email account for additional information as instructed when you submitted your application.
Step 6: Submit Supplemental Application Forms.
Applicants will be provided supplemental forms with their approval to apply email based on their clinical affiliate site.
NOTE: This program is authorized, exempt, or not subject to state regulatory compliance and may enroll students from all 50 states
Note to Florida Residents: Indiana University histotechnology Program is a Florida Approved Training Program. If you are accepted into the program and are a Florida resident, you will be provided instructions to obtain your "clinical laboratory trainee license".
Note to New York Residents: The state of New York does not approve out of state programs. Graduates of our associate degree may meet the NY requirements. You will be required to work with the New York State Education Department: http://www.op.nysed.gov/prof/clt/clp-cytlic.htm to request licensure.
NOTE: Supplemental Application Materials received or postmarked after the program’s deadline (May 7th) are considered late. Incomplete or late applications are only considered once completed and only after all completed/on- time applications have been reviewed and if positions in the program are still available. Clinical Affiliates: The distance education program in histotechnology is offered through the Indiana University School of Medicine and functions as a cooperative effort between IU School of Medicine and qualifying histology laboratory nationwide. While most students are employed in the facility where they complete the practicum, our clinical affiliates sites are based on need and can accommodate the non-employed student.
